REQUIRED INFORMATION FOR A COURT ORDER TO SUPERIOR OR ONTARIO COURT

In order to order a transcript from an Ontario or Superior Court proceeding you must have the following information:

NAME OF THE PROCEEDING

COURT LOCATION

JUDGE'S NAME AND/OR COURTROOM

DATE OF PROCEEDING

Without this required information the order form would be returned by the records management office at the courthouse.
